NIDDK-CR Data and Resources Use Agreement (DUA) - Example Template [

The DUA is legally binding and signed by the requesting Principal Investigator and institution official agreeing to adhere to the outlined terms of use for the requested resources. This form is an EXAMPLE only and should not be completed and sent to NIDDK-CR. A customized DUA will be generated and routed to you for signature after the request is approved.
